Understand the details before care begins.

Rise accepts all insurance providers, including Select Health. This guide helps you ask better questions and understand the terms that can affect your cost and access.

Verify your specific plan

One carrier can have many different plans.

Rise accepts all insurance providers, including Select Health. Network participation, referrals, authorizations, visit limits, and patient cost can still vary by employer and plan.

Call Rise with your insurance information before the first appointment. The clinic can help identify the right questions, but your insurer makes the final coverage determination.

A simpler process

Know what to confirm before visit one.

01

Confirm the network

Ask about Rise Physical Therapy for your exact plan—not only the insurance company name.

02

Review your share

Check your remaining deductible, visit copay or coinsurance, and out-of-pocket status.

03

Check plan rules

Ask whether a referral, authorization, or visit-limit review applies to outpatient physical therapy.

Plain-language guide

Insurance terms without the jargon.

These are general definitions. Your plan documents and insurer determine how each term applies to you.

Deductible
The amount you pay for covered care before your plan begins paying according to its terms.
Copay
A fixed amount your plan may require for each covered visit.
Coinsurance
A percentage of the allowed cost that you may owe after the deductible is met.
Out-of-pocket maximum
The most you pay for covered, in-network services during a plan year, subject to your plan’s rules.
In network
A provider has a contract with your plan. Network status may differ by product even under the same insurer.
Prior authorization
Approval your plan may require before it covers some or all visits.
Visit limit
A cap on covered visits. Limits may be annual, diagnosis-specific, or shared with other therapy services.
Explanation of benefits
A statement from your insurer showing what was billed, what the plan processed, and what you may owe. It is not a bill.

  • Do I have a deductible, and how much remains?
  • What is my copay or coinsurance for outpatient physical therapy?
  • Does my plan require a referral or prior authorization?
  • Does my plan limit physical therapy visits or require review after a certain number?
  • Are there different rules for work, auto, or post-surgical care?
